Ultimate Difficulty
Start off by beating the game and then go back and play the "A New Day Chapter". Then go to the Shiba-Q headquarters (3rd floor) and then you can buy it. Yup, you can buy ultimate difficulty as a quest item!
Chapter Select
The chapter select is on the phone main menu, and once you beat the entire game, you can go back and select which chapter you want to play.
Secret Ending
You can unlock the secret ending scene by collecting all of the secret reports and then defeating the final boss of the main story.
